Atrium Physiotherapy and Performance (“we,” “us,” or “our”) is committed to protecting the privacy and confidentiality of your personal information. This Privacy Policy describes how we collect, use, disclose, and safeguard information in connection with the physiotherapy services we provide.
This Policy is governed by the Personal Information Protection Act (PIPA) of British Columbia and, where applicable, the federal Personal Information Protection and Electronic Documents Act (PIPEDA).
1. Information We Collect
We collect personal information that is necessary to provide you with safe and effective physiotherapy care. This may include:
- Identifying information: Full name, date of birth, home address, telephone number, and email address;
- Health information: Medical history, current diagnosis, injury or condition details, medications, surgical history, and clinical notes from your treatment;
- Insurance and billing information: Extended health benefit plan details, policy numbers, and billing records; and
- Appointment records: Booking history, attendance, and communications related to your care.
We collect this information directly from you, your authorized representative, or (with your consent) from other healthcare providers involved in your care.
2. Purpose of Collection
We collect and use personal information only for the following purposes:
- To assess, diagnose, treat, and monitor your physiotherapy condition;
- To communicate with you regarding appointments, treatment plans, and follow-up care;
- To submit claims to and receive payment from your insurer, including ICBC, WorkSafeBC, and private extended health plans;
- To coordinate your care with other health professionals, where you have given consent or where otherwise permitted by law;
- To comply with legal, regulatory, and professional obligations; and
- To improve the quality and safety of the services we provide.
We will not use your personal information for any purpose other than those listed above without first obtaining your consent, except where permitted or required by law.
3. Disclosure of Personal Information
We do not sell, rent, or trade your personal information. We may disclose your information only in the following circumstances:
- With your consent: For example, when referring you to or communicating with your family physician, specialist, or another healthcare provider;
- Insurance and billing: To third-party insurers, WorkSafeBC, or ICBC to process claims on your behalf;
- Legal requirements: Where we are required by law, court order, or a regulatory body to disclose information; and
- Safety: Where disclosure is necessary to prevent or reduce a serious and imminent risk to the health or safety of any individual.
4. Retention of Records
Clinical records are retained for a minimum of 16 years from the date of last entry, or until a minor patient reaches the age of 19, whichever is longer, in accordance with the requirements of the CPTBC and applicable legislation. Records may be retained for longer periods where required by law or professional standards.
5. Security
We implement physical, administrative, and technical safeguards appropriate to the sensitivity of the information held to protect against unauthorized access, use, disclosure, alteration, or destruction. These include:
- Secure, access-controlled storage of paper records;
- Password-protected and encrypted digital systems; and
- Limiting access to personal information to staff members who require it to perform their job functions.
No method of transmission or electronic storage is entirely secure. While we strive to protect your information, we cannot guarantee absolute security.
6. Your Rights
Subject to applicable law, you have the right to:
- Access the personal information we hold about you and receive a copy upon written request;
- Correct inaccurate or incomplete information in your records;
- Withdraw consent to the use or disclosure of your information for non-essential purposes at any time, subject to legal and contractual restrictions; and
- Request deletion of your personal information, subject to our legal and regulatory retention obligations.

7. Cookies and Website Usage
Our website may use cookies or similar tracking technologies to improve user experience and analyze site traffic. No personal health information is collected through our website. You may disable cookies through your browser settings, though this may affect certain website functionality.
8. Changes to This Policy
We may update this Privacy Policy periodically to reflect changes in our practices or applicable law. The updated Policy will be posted on our website with a revised effective date. We encourage you to review this Policy regularly. Your continued use of our Services following any updates constitutes your acceptance of the revised Policy.
